Intake Forms:

Hurt Craft Deep Tissue Therapy and its associated LMTs require intake forms to be submitted from all clients before service is rendered.
We will also periodically require return clients to update their intake forms. This is not an easy, fun, relaxing, spa time. We practice intense and corrective deep tissue therapy that is not suitable for everyone. We absolutely require a simple intake form to assess your medical history and any contraindications that may present BEFORE we lay aggressive hands on you. It is your responsibility to provide all necessary information so that we can best treat you. Keeping pertinent medical information from us? Not cool. Intake forms provide legal protection to us and our team in the instance you don’t want to put any real information on there. Regardless, we have the right to refuse service to anyone at any time and this may result in a charge in accordance with our cancellation policies.

Cancellation Policy:

We require 24-hour notice to reschedule or cancel your appointment. Cancellations after the 24-hour window may be charged a 50% Late Cancellation Fee. No Call/No Show appointments may be charged 100% of the appointment value. Your appointments are reserved specifically for you and are difficult to fill without proper notice. As professionals, we enforce policies to protect our time, effort and income.

Zero Tolerance Policy:

Hurt Craft Deep Tissue Therapy has a strict Zero Tolerance Policy against physical, verbal or sexual harassment, hate speech, threatening behavior or  just being a jerk in general. This policy is absolutely enforced and will result in immediate termination of your appointment with a 100% charge of service as well as a non-negotiable ban from our premises. Legal action will be taken if necessary.
